Damian N. Dell'Anno (Chairman)

Mr. Dell'Anno is Senior Executive Vice President and Chief Operating Officer for Formation Capital, a leader in equity investment in senior housing and care. Prior to Formation Capital, Mr. Dell'Anno was with Harborside Healthcare as Chief Operating Officer since 1994, President since 2001, and Chief Executive Officer and Chairman in May, 2005. He has served as head of Harborside's specialty services group and was instrumental in the initial development of Theracor, Harborside's rehabilitation therapy company. Mr. Dell'Anno also was responsible for the supervision and coordination of all of Harborside's reimbursement-related matters until 1994. Prior to joining the company, Mr. Dell'Anno served as director of budget, reimbursement, and cash management for the Mediplex Group, Inc., from 1988 to 1989. From 1985 to 1988, he was payment systems director for Carney Hospital, a 400-bed acute-care facility in Boston, Massachusetts. Before that, he was manager of accounting at Brockton Hospital and a Medicare auditor for Blue Cross Blue Shield of Massachusetts. Mr. Dell'Anno has a B.S.B.A. degree in accounting from Suffolk University.

Gary Duty

Mr. Duty is an experienced healthcare industry executive with over 20 years of experience in the institutional pharmacy industry. He is co-founder and former partner of Brown & Duty Inc., which owned and or managed four institutional pharmacies in Western Pennsylvania. He was a partner in HPS, a western Pennsylvania-based institutional pharmacy that was acquired by Gatti LTC Services Inc. in 1990. He was instrumental in the success of Gatti LTC Services Inc. as it grew to service over 25,000 beds in Pennsylvania and contiguous areas. Mr. Duty served in various marketing and operating capacities as a Gatti LTC executive and subsequently served in similar roles with American Medserve and Omnicare. He has extensive experience with the complex customer acquisition process that characterizes the marketing of pharmacy services to the long term care industry as well as the mechanics of compliance with regulatory standards. Mr. Duty is a Registered Pharmacist and a member of the American Society of Consultant Pharmacists.

Tamara Elias, M.D.

Dr. Elias joined Essex Woodlands Health Ventures after 5-1/2 years at McKinsey & Company, a global management consulting firm. As a senior engagement manager in McKinsey's New York office, Dr. Elias advised primarily pharmaceutical and diagnostic companies in strategic, operational, and business development areas. She managed numerous McKinsey and client teams, developing and delivering recommendations to senior management. Prior to joining McKinsey, Dr. Elias was a general surgery resident at Massachusetts General Hospital in Boston and received a national grant during a research fellowship in plastic and reconstructive surgery at New York University.

Dr. Elias graduated summa cum laude and Phi Beta Kappa with a Bachelor of Arts degree in the dual majors of Biology and Anthropology from Yale University. She obtained her M.D. degree from Johns Hopkins School of Medicine, graduating with AOA honors.

Myles D. Greenberg, M.D.

Dr. Greenberg joined CHL Medical Partners in 2004. Previously, he was a Senior Principal at A. M. Pappas & Associates and held a faculty appointment as an Assistant Professor at the University of North Carolina School of Medicine. Prior to becoming a venture capitalist, Dr. Greenberg was Assistant Clinical Director and Instructor of Emergency Medicine in the Department of Emergency Medicine at Beth Israel Deaconess Medical Center, Harvard Medical School. His professional affiliations include being a Diplomat with the American Board of Emergency Medicine and a Fellow of the American College of Emergency Physicians. Dr. Greenberg currently serves on the board of two portfolio companies, Millennium Pharmacy Systems, Inc. and Oculir, Inc., and has previously served on the boards of Angiomics, Dynogen, FlowCardia, and NuVasive. He was also an observer member of the boards of ArgoMed, LipoScience, and Syntonix. Dr. Greenberg received his B.A.S. from the University of Pennsylvania School of Engineering and Applied Science. He holds an M.D. from the Yale University School of Medicine and an MBA from the Harvard Business School.

Jonathan Perl

Mr. Perl has more than 8 years of investing experience, focused primarily on early-stage information technology and life science companies. He led the BVIV investments in iLumin Software, Rannoch and Millennium Pharmacy Systems. ILumin was sold to Computer Associates (NYSE: CA) in October 2005. He was a Partner in BVIV and will be a General Partner in BV V. Mr. Perl joined Intersouth Partners in 1997 as a member of the third class of Kauffman Fellows. In 1999, he became a Partner. He was responsible for the sourcing, due diligence, structuring and negotiation of more than a dozen early stage investments, including several highly successful companies. From 1991 to 1995, Mr. Perl was the Director of Washington Operations for IICC, a placement agent, where he was responsible for a variety of consulting arrangements with both investment management firms and venture backed companies. Prior to joining IICC, Mr. Perl was in the management development program at American Security Bank (now BofA). Mr. Perl holds an MBA from the Amos Tuck School of Business at Dartmouth College (1997) and a BA (magna cum laude) in classical history from Tufts University (1988).

Mark Pacala

Mr. Pacala has 25 years of general management and venture capital experience in services, technology and healthcare companies. He began his career as a banker in 1977 at Manufacturers Hanover Trust Corp. and transitioned to strategic planning in healthcare at Booz, Allen and Hamilton. He served as Director of Corporate Planning and Vice President of Operations at Marriott Corporation from 1984 to 1989, where he founded the Fairfield Inns division. From 1989 to 1994, Mr. Pacala was a Senior Vice President and General Manager at Walt Disney Company, where he founded the Disney Vacation Club. He then served as the CEO of Forum Group, Inc., a public, $200-million-revenue senior housing and healthcare company, which later was sold to Marriott. In 1997, Mr. Pacala was recruited by Essex Woodlands to serve as the CEO of American WholeHealth, an integrative health network company which combines conventional medicine, alternative medicine, nutrition and wellness. He joined Essex Woodlands full-time in 2002 and manages investments in and/or sits on the Board of Directors of BENU, CareGuide (formerly Coordinated Care Solutions,) KeyHealth, Touchstone Health, and SleepMed. Mr. Pacala holds a Bachelor of Arts from Hamilton College, where he graduated magna cum laude and Phi Beta Kappa, and a Masters of Business Administration from Harvard Business School, where he graduated with distinction.

Richard Scardina

Mr. Scardina is a proven healthcare services executive with experience in rapid growth and innovation in businesses including pharmacy benefit management, disease management, infusion therapy and specialty pharmacy. He has been a Director of Millennium since October 2007 and Chief Executive Officer beginning September 2008. Prior to that Mr. Scardina was a private investor and business consultant primarily to healthcare related organizations. From July 1989 until May 2006 he was employed by Caremark Rx and its predecessor companies, most recently as Executive Vice President of Operations. Before that, Mr. Scardina was an employee of Price Waterhouse for thirteen years. He was also previously Chairman and an Executive Committee Member of the Pharmacy Care Management Association, the trade association for the pharmacy benefit management industry.

Mr. Scardina earned a Bachelor of Accounting Science Degree with High Honors from the University of Illinois and is a Certified Public Accountant.
